Job Summary

Develops & oversees strategies to manage and improve the healthcare and outcomes of SJMC populations moving from hospitalization and clinics to home. Responsible for the planning, priority setting, coordination, implementation, and evaluation of programs related to patient care coordination and throughput. Collaborates with providers on appropriate utilization of resources, including LOS and readmissions. Facilitates seamless transitions of care within the healthcare system. Will be a leader in driving clinical care transformation, delivering a comprehensive interdisciplinary approach to achieve care excellence throughout the patient care continuum that measurably improves quality, patient care experience, and reduces healthcare costs. 

Essential Functions

1. Oversees SJMC patient care coordination function:

  • Provides appropriate training, education and management of the department operations and employees in order to provide completed responsibilities, managing caseloads, interpreting regulations, policies, operational procedures, objectives and appropriate care plans and interventions 
  • Utilizes integrated care principles and best evidenced based clinical care to guide department servcies

2. Collaborates with other providers and community programs:

  • Participates with other program and departmental directors/providers/clinics/homehealth in policy development and strategic planning

3. Manages day-to-day operations of department

  • Accountable for appropriate utilization of staffing, supplies and other resources as appropriate for patient care needs and budget
  • Conducts employee performance reviews, disciplinary action, and supervision
  • Adheres to practices and ensures compliance with SJMC Corporate Compliance Program

 

Job Requirements

Minimum Education

Required: Bachelor's degree in Nursing or Social Work.

Preferrred: Master's degree in: Nursing, Social Work, Health Administration, or Business Administration.

Minimum Work Experience

Required: Recent experience in care management. Experience in leading new initiatives. Ability to work with groups and lead groups in change processes. Well-developed verbal and written communication skills. Strong analytical skills. Computer skills. Knowledge of federal, state, regulatory agency and local healthcare related laws and regulations.

Preferred: Five years of progressive management experience in a healthcare related organization. Hospital managed care experience. Current knowledge of health care economics, trends, and reimbursement.
