Responsible for ensuring that patient care is coordinated and managed appropriately, and for ensuring that care and services are delivered appropriately. Supervises clinical personnel and the maintenance of quality client care in compliance with federal state and local regulations and accreditation standards.

Essential Functions

  1. Coordinates clinical staffing
    Receives case referrals. Reviews available patient information related to case, including home visits, to determine hospice/home care needs. Assigns appropriate hospice/home care personnel to cases as needed, and responds to case load with effective utilization of staff including assignment and reassignment of duties. Conferences with department director and medical director regarding any questions about an individual's eligibility for services. Attends case conference meetings to facilitate coordination of care and discussion of interdisciplinary group involvement, or assigns appropriate staff.
  2. Provides clinical supervision
    Responsible for the supervision of clinical staff and the maintenance of quality patient care under federal, state, and local regulations and accreditation standards. Reviews patient's medical diagnosis, prognosis, medications, procedures and clinical course. Monitors clinical documentation of completeness and accuracy and assures that it meets standards for good clinical documentation and successful billing. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care. Complies with accepted professional standards and principles.
  3. Acts as resource for staff
    Assists the department director in the planning, implementation and evaluation of in-service and continuing education programs. Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, goals and objectives, both short and long range. Assists with the orientation of new organization personnel, directly or delegated. Participates actively in quality assessment performance improvement activities.
  4. Provides direct patient care
    Available to perform all functions of a Home Care RN as needed.

Education & Work Experience
Minimum
Associate Degree in Nursing. Wyoming RN liscense. 1-2 year of Home Health or Hospice experience or 2 plus years of nursing management experience

Preferred 
Bachelor Degree in Nursing

Working Conditions
70 percent of the job will be office conditions
30 percent will be out in the field taking care of patients wherever they call home
Need reliable transportation for all weather conditions

Physical Requirements
In an eight (8)-hour workday:
sitting for 6 hours, standing for 1 hour, walking for 1 hour
Be able to lift and effectively transfer 75 pounds.
